This week's brief at a glance:

AREDS2 is a specific 6-ingredient supplement formula tested by the National Eye Institute in 4,203 adults aged 50 to 85. It reduces the risk of progression from intermediate to advanced age-related macular degeneration by about 25 percent (NEI, 2024)

AREDS2 only helps if you already have intermediate AMD or advanced AMD in one eye. It does not prevent AMD from starting and does not help early AMD (NEI, 2024)

The 2022 NEI 10-year follow-up showed the lutein-and-zeaxanthin version reduced progression another 20 percent compared to the older beta-carotene formula. Smokers must avoid the original beta-carotene version due to lung cancer risk (NEI, 2022)

Age-related macular degeneration is the leading cause of vision loss in adults over 65. It does not cause blindness in the traditional sense. Instead, it erases the center of your visual field while leaving the edges intact. Reading, recognizing faces, and driving become difficult or impossible. There is no cure. But there is one supplement formula that the National Eye Institute has proven, in two large clinical trials, can slow it down.

That formula is AREDS2. Most people who hear about it assume it is just "eye vitamins" and pick something off the pharmacy shelf labeled as such. The specifics matter. AREDS2 is six specific ingredients at six specific doses, and substitutes do not deliver the same protection.

What AREDS2 Actually Contains

The current AREDS2 formula, as defined by (NEI, 2024), contains vitamin C 500 mg, vitamin E 400 IU, lutein 10 mg, zeaxanthin 2 mg, zinc 80 mg (or 25 mg low-dose version), and copper 2 mg (to prevent zinc-induced copper deficiency).

Lutein and zeaxanthin replaced beta-carotene from the original 2001 AREDS formula. Beta-carotene was effective but raised lung cancer risk in current and former smokers. The lutein-and-zeaxanthin version is safe across smoking status and proved at least as effective overall, and slightly more effective in the long-term follow-up.

Who Actually Benefits

AREDS2 is not a vision-protection supplement for the general public. It is specifically tested and approved for adults with intermediate AMD in both eyes, or intermediate AMD in one eye with advanced AMD in the other. According to (NEI, 2024), the supplements have not shown benefit in people without AMD, or with only early AMD.

The 25 percent risk reduction is the relative risk over 5 years. Translated to absolute numbers, that means roughly 1 in 4 people who would have progressed to vision-threatening AMD instead held stable. The benefit is real, but it requires you to be in the eligible group, and the only way to know that is an eye exam with a retinal evaluation.

The 10-Year Follow-Up Confirmed the Lutein/Zeaxanthin Edge

(NEI, 2022) published the 10-year follow-up of AREDS2 participants. Those originally assigned to lutein and zeaxanthin had an additional 20 percent lower risk of progression to late AMD compared to those originally assigned to beta-carotene. The longer the follow-up window, the more clearly the lutein-and-zeaxanthin advantage emerged.

The data also showed that smokers who took the original beta-carotene formula had a higher lung cancer risk, confirming the rationale for the switch. If you are reading the label on a bottle and see beta-carotene listed, that is the older formula. Look for AREDS2-specific products with lutein and zeaxanthin.

What AREDS2 Cannot Do

AREDS2 does not prevent AMD from developing. People without AMD do not benefit from taking it. AREDS2 does not reverse advanced AMD. Once the disease has progressed to the advanced stage in both eyes, AREDS2 has no role. AREDS2 is not a substitute for the regular ophthalmology monitoring that intermediate-AMD patients need to track progression and decide on additional interventions like anti-VEGF injections if neovascular (wet) AMD develops.

For more on age-related screening that actually moves the needle, see our piece on the 5 lab numbers your doctor should track after 50.

Picking the Right Bottle

Look for the AREDS2 label on the front of the bottle. Common formulations include PreserVision AREDS2 (Bausch + Lomb), the Ocuvite line, and several pharmacy private-label equivalents. Check the back label for the specific ingredient amounts: lutein 10 mg, zeaxanthin 2 mg, zinc 80 mg or 25 mg, copper 2 mg, vitamin C 500 mg, vitamin E 400 IU. If those numbers do not match, it is not AREDS2.

No. AREDS2 was not tested for prevention, and the data so far does not support a preventive benefit in people without AMD. Family history is a reason to start dilated retinal exams earlier than the general 60-and-up guideline, not a reason to start the supplement.

Mostly yes. The 80 mg zinc in the standard AREDS2 dose can interact with certain antibiotics (tetracyclines, fluoroquinolones) by reducing their absorption. Take antibiotics 2 hours apart from AREDS2. High-dose vitamin E may interact with blood thinners. Tell your doctor what you are taking.

Spinach, kale, and corn supply lutein and zeaxanthin in food form, which is good for general eye health.
Dry AMD (about 85 percent of cases) is gradual thinning of the macula. Wet AMD is the sudden growth of abnormal blood vessels under the retina that leak fluid and cause rapid vision loss. AREDS2 reduces progression to advanced dry AMD and reduces conversion to wet AMD. Wet AMD itself is treated with anti-VEGF injections, not supplements.
