Glucosamine Failed Most Knees. Not the Ones Hurting Most.

We have said before that glucosamine works no better than a sugar pill for most knees, and the big trial still says so. What got buried is the exception: in the NIH funded GAIT trial, published in the New England Journal of Medicine, the one group that separated from placebo was the people whose knees hurt the most. In that subgroup, roughly 79 percent got meaningful relief on the glucosamine and chondroitin combination against 54 percent on placebo.

So the useful question is no longer whether glucosamine works. It is which knee you actually have.

The Short Answer

Mild knee aches, no. Moderate to severe pain, the picture changes.

  1. Overall the trial was negative. Across 1,583 adults, NCCIH reports that glucosamine, chondroitin and the two together did not beat placebo for the group as a whole.
  2. One subgroup did separate. In the moderate to severe pain arm, the combination outperformed placebo, and the researchers themselves called that finding preliminary.
  3. The dose and the pairing matter. The result belongs to glucosamine taken with a partner compound at trial level amounts, not to a token sprinkle on a label.

What the Trial Actually Found

GAIT randomized 1,583 adults with knee pain across five arms for 24 weeks. For everyone pooled together, nothing beat placebo, which is where the popular verdict comes from and why we published it.

But the researchers had split participants in advance by pain severity, and in the moderate to severe group the glucosamine and chondroitin combination pulled clear. It was a smaller slice of the study, so NCCIH regards it as a signal to confirm, not a settled result. A signal is still more than the mild-pain group got, which was nothing.
