This week's brief at a glance:

HGH can modestly increase lean body mass and decrease body fat in healthy older adults, but does not improve strength, energy, or lifespan (Mayo Clinic, 2024)

Up to 30% of HGH users experience side effects including fluid retention, joint pain, carpal tunnel syndrome, and high blood sugar (Harvard Health, 2023)

Using HGH for anti-aging is not FDA-approved and is prohibited under federal law in the United States (Cleveland Clinic, 2024)

The HGH pitch is seductive. A few injections a week, and the promise is more muscle, less fat, better skin, sharper memory, deeper sleep. Anti-aging clinics charge thousands a month for it. Celebrities credit it for their 50-year-old physiques.

What the actual evidence shows is more complicated. Human growth hormone changes body composition modestly in healthy older adults, but does not deliver the strength, energy, or longevity benefits the marketing implies. The side effect rate is real. And in the United States, prescribing HGH for anti-aging is illegal.

What HGH Actually Does in Aging Adults

Modest changes, oversold: In studies of healthy older adults, HGH treatment increases lean body mass and decreases body fat. The gain in muscle does not translate into gains in strength. Bone mineral density barely changes (Mayo Clinic, 2024).

The visible aesthetic effect (slightly less fat, slightly more muscle) is real but small compared to what the marketing suggests. Most people would not see the body composition change without a DEXA scan.

There is no convincing evidence that HGH improves cognition, energy, or skin quality in healthy adults at standard doses. The studies that exist are small, short, and mixed.

The Side Effects the Brochures Don't Mention

A predictable cluster shows up in roughly 1 in 3 users: Up to 30% of HGH users experience side effects (Harvard Health, 2023). The most common are fluid retention (swelling in the hands and feet), joint and muscle pain, carpal tunnel syndrome, and high blood sugar.

Higher doses can produce gynecomastia (breast tissue growth) in men, insulin resistance progressing to type 2 diabetes, and worsened sleep apnea.

There is also a theoretical, debated risk of increased cancer growth in people with undiagnosed early cancer, because HGH and IGF-1 drive cell growth. The risk is not clearly proven but is the basis of most caution flags.

Why Lifespan Isn't on the List

The longevity headline does not match the longevity data: In animal research, the opposite of the anti-aging pitch shows up. Animals with lower growth hormone signaling tend to live longer, not shorter. Mice with mutations that block growth hormone have some of the longest lifespans on record.

Humans with naturally low growth hormone (Laron syndrome) have low rates of cancer and diabetes despite their short stature. Studies of older HGH users have not shown lifespan benefits and in some cases suggest mortality increases.

The takeaway is uncomfortable for anti-aging marketing: the science of growth hormone and longevity points the wrong direction for the way it is being used.

Who Actually Benefits

Replacement is appropriate in true deficiency: HGH treatment is FDA-approved for documented growth hormone deficiency in children, and in adults with pituitary disease, severe HIV-associated wasting, or short bowel syndrome (Cleveland Clinic, 2024).

In these patients, the deficiency is real and the benefit is large. Growth hormone deficiency in adults causes fatigue, low muscle mass, increased fat, and decreased exercise tolerance. Replacement reverses these.

The line between "deficiency" and "low normal" is what gets exploited by anti-aging clinics. A lab number near the bottom of the range is not the same as a clinical deficiency. The diagnosis requires specific stimulation testing and pituitary imaging.

What Works Better Than HGH

The boring answers beat HGH at most goals: For body composition, resistance training plus adequate protein intake. The lean mass and fat loss from twelve weeks of progressive resistance training match or exceed what HGH delivers, with no side effects.

For energy, address sleep, dietary patterns, and underlying conditions like thyroid disease or depression. For skin quality, sun protection, retinoids, and the basics of dermatology. For sexual function, manage cardiovascular risk and, when appropriate, consider testosterone replacement under medical supervision.

The marketing of HGH succeeds because it promises a single solution to several aging problems. The actual solutions are several different habits, each with stronger evidence and lower risk.

No. In the United States, using HGH for anti-aging or athletic enhancement is prohibited by federal law. It is legal only for FDA-approved indications like documented growth hormone deficiency, pediatric short stature in certain cases, and a small set of medical conditions.

Most have weak evidence. Adequate sleep, resistance training, and not eating large meals right before sleep produce modest, transient rises in natural HGH. Pills marketed as HGH boosters rarely outperform these basics in clinical trials.

Modestly, but resistance training does the same thing more reliably. Is IGF-1 the same as HGH?

Not exactly. Growth hormone is released by the pituitary and stimulates the liver to produce IGF-1, which mediates most of HGH's effects. Some treatments target IGF-1 directly. Higher IGF-1 levels carry the same theoretical cancer-growth concerns.
