<p>Your annual physical comes with a standard lab panel. Most of it is useful. Some of it is incomplete. Adults over 50 who want a genuinely predictive picture of their cardiovascular and metabolic health need five specific numbers beyond the cholesterol total, and only some of them appear on the default lipid order.</p>
<p>Here is what each one tells you, why it matters, and what to ask for at your next visit.</p>
<h3>1. ApoB (Apolipoprotein B)</h3>
<p>ApoB measures the actual number of cholesterol-carrying particles in your blood. LDL cholesterol measures how much cholesterol those particles contain. The distinction matters because two people can have identical LDL values but very different particle counts, and the particle count is what physically deposits in artery walls.</p>
<p>According to the (<a href="https://www.nia.nih.gov/news/topics/biomarkers" target="_blank" rel="noopener">NIH National Institute on Aging, 2024</a>), ApoB is increasingly recognized as a better predictor of cardiovascular events than LDL alone, particularly in adults with metabolic syndrome or insulin resistance. A target of under 90 mg/dL for healthy adults, under 80 for those with established cardiovascular disease, is the current consensus.</p>
<h3>2. hs-CRP (High-Sensitivity C-Reactive Protein)</h3>
<p>hs-CRP measures inflammation in your blood. Chronically elevated inflammation drives atherosclerosis, contributes to insulin resistance, and is associated with most age-related diseases. Mayo Clinic notes that hs-CRP is one of the few inexpensive blood tests that captures whole-body inflammation in a meaningful way (<a href="https://www.mayoclinic.org/tests-procedures/c-reactive-protein-test/about/pac-20385228" target="_blank" rel="noopener">Mayo Clinic, 2024</a>).</p>
<p>Healthy values sit under 1.0 mg/L. Levels of 1 to 3 mg/L indicate moderate cardiovascular risk. Above 3 mg/L is a strong signal to address inflammatory drivers: sleep, diet quality, dental health, abdominal fat, and chronic stress.</p>
<h3>3. Fasting Insulin</h3>
<p>Insulin rises before blood sugar does. By the time HbA1c is elevated, the pancreas has often been overproducing insulin for a decade. Fasting insulin catches insulin resistance years earlier than the standard glucose-based tests, which makes it one of the most useful early-warning markers for metabolic disease.</p>
<p>Cleveland Clinic considers a fasting insulin under 10 µIU/mL optimal for most adults, with anything above 15 suggesting meaningful insulin resistance (<a href="https://my.clevelandclinic.org/health/diagnostics/22515-insulin-test" target="_blank" rel="noopener">Cleveland Clinic, 2024</a>). The test is inexpensive but rarely ordered unless you ask.</p>
<h3>4. HbA1c</h3>
<p>HbA1c measures your average blood sugar over the past three months. It is the gold standard for diagnosing diabetes but it is also informative below the diagnostic cutoff. Levels of 5.7 to 6.4 percent indicate prediabetes, where intervention can prevent progression. Above 6.5 percent is diabetes.</p>
<p>Most lab panels include HbA1c for adults over 45, but many do not. Ask explicitly. The combination of HbA1c plus fasting insulin gives a much fuller picture than either alone.</p>
<h3>5. eGFR (Estimated Glomerular Filtration Rate)</h3>
<p>eGFR estimates kidney function from a routine blood creatinine reading. Kidney function declines slowly with age, and early stages of chronic kidney disease are typically silent. Catching reduced eGFR early lets you adjust medications, manage blood pressure aggressively, and avoid contrast dye exposure that could accelerate decline.</p>
<p>Adults over 50 should have eGFR checked annually. A value above 60 mL/min/1.73 m² is generally healthy. Below 60 sustained for three months indicates chronic kidney disease and warrants a conversation about causes and management.
