The <a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC6953387/" target="_blank" rel="noopener">2019 Cochrane review of acetyl-L-carnitine in diabetic peripheral neuropathy</a> pooled the randomised evidence and reached a hedged conclusion: adverse events may be no more common than with placebo, but the evidence on harms is too uncertain to judge safety, and the included trials were funded by or advised by manufacturers.</p><p>That is a fair description of the whole compound. There is a mechanism, there are trials, and there is not a clean verdict. Anyone selling you certainty here is ahead of the evidence, in either direction.</p><div class="callout"><div class="n">The honest limit</div><div class="d">The <a href="https://www.nia.nih.gov/health/brain-health/cognitive-health-and-older-adults" target="_blank" rel="noopener">National Institute on Aging</a> states that no vitamin or supplement is recommended for preventing Alzheimer's disease or cognitive decline. The people with a measurable shortfall are usually the ones with a medical reason: certain genetic disorders, dialysis, or specific medicines. Those are clinician situations, not shelf situations.</p><p>The everyday version of a gap is narrower and duller.
